Q. Where is NYU Division of Vascular and Endovascular Surgery located?

A. NYU Division of Vascular and Endovascular Surgery is conveniently located at the NYU Langone Medical Center at 530 First Avenue, Suite 6F, New York, NY 10016. Please visit Contact Us for detailed map and public transportation information.

Q. What are NYU Division of Vascular and Endovascular Surgery office hours?

A. Office hours are Monday through Friday 9:00 AM to 5:00 PM by appointment. An NYU Vascular surgeon is on call 24 hours a day, 7 days a week for emergencies.

Q. Can I email my doctor?

A. Not at the present time. You can however email the physician's secretary for an appointment. Please be sure to get the individual secretary's business card with her email address at the end of your visit.

Q. How can I make an appointment?

A. If you are a new patient please call (212) 263-7311 and follow the prompts. If you are an established patient, please call the physician's secretary directly so that she can schedule an appointment for you. If you reach a secretary's answering machine, please leave a detailed message and someone will return your call within 1 business day. If you need an emergency appointment please call the telephone number above again, follow the prompts and have the physician's secretary paged.

Q. What if I need to cancel my appointment?

A. You may call our office at the general number or the physician's secretary directly to cancel your appointment. Cancellations less than 24 hours prior to the appointment may be subject to a cancellation charge. Please note that insurance carriers do not reimburse for missed appointments. Please reference our financial policy for specifics.

Q. What if I don't have a doctor?

A. Our office staff is happy to assist you in finding an NYU Vascular surgeon who can meet your needs. For a complete listing and more information on our surgeons, also visit the "Providers" section of our website.

Q. Do I need a referral?

A. Every insurance plan is different and it is your responsibility as the patient to be familiar with your plan's requirements, and covered benefits. Contact your insurance carrier if you are unsure about these benefits. If you come to your appointment without a referral and one is required by your insurance carrier, you assume financial responsibility for the service. Some referring physician offices require several days to process a referral so please allow adequate time for this.

Q. How can I get a refill on my medication?

A. You can speak to your doctor's assistant to request a refill for your medications that were prescribed. Please request prescription refills during regular office hours so that we can contact your pharmacy during normal business hours. Please allow 24-48 hours for processing the request.

Q. How do I obtain my test results?

A. Please contact your physician's secretary for test results.

INSURANCE

Q. What insurance plans do you accept?

A. At NYU Division of Vascular and Endovascular Surgery we participate in many managed care plans and see patients with traditional insurance. See the detailed list of plans below for your reference. Many plans require that you make a copayment at the time of your visit. To make it as easy as possible for you, we accept cash, checks, and most major credit cards at our office.

Some insurance plans may require that you get authorization from them before you visit us. Since this differs by plan, please check with your insurance company so you can receive complete benefits from your coverage. We also have trained in-house specialists who can answer many of your insurance questions.

Our practice participates with the following plans. To find out if a particular physician or clinician participates with your plan, please contact our office or your insurance company. Please note that this list is subject to change. Please confirm all provider information prior to each visit. Please verify your eligibility and coverage. You will be asked for your insurance card at each visit. If you have insurance coverage from a company other than what is listed, services are probably out of network and you will be responsible for a greater out-of-pocket expense.

MEDICAL RECORDS

Q. How can I obtain a copy of my medical records?

A. You may request your medical records by sending us an authorization allowing us to release your private health information. You must mail the form to:

By law, we may not release your medical information through a request made by phone or by e-mail.

The request must include your name, date of birth and exactly what records you are requesting in order to process your request. Attorneys and Life Insurance Companies should contact our office for the current copying fees for obtaining medical records.
